1. General Conditions

  • The "Smile Doctor Ambassador Club" referral program is operated by Smile Doctor, Mona Vale.
  • By participating in this program, presenting the referral card, or receiving an account credit, both the Referrer (Ambassador) and the Referred Patient agree to these Terms and Conditions.
  • This program strictly complies with the Health Practitioner Regulation National Law (NSW). No reward is provided in exchange for a public testimonial or review.

2. The Referred Patient Offer ($200 Off First Comprehensive Exam)

  • Definition: "First comprehensive exam" refers to an initial consultation which may include, subject to clinical necessity, a comprehensive oral examination (ADA Item 011), intraoral radiographs (Item 022), OPG (Item 037), clinical photographs (Item 071 and 072) and TMJ and occlusion analysis (item 963).
  • Deduction: The $200 discount will be deducted from the total standard, non-discounted fee of the aforementioned services.
  • Limit: If the total cost of the initial appointment is less than $200, the fee will be reduced to $0. No cash "change" or residual credit will be provided to the Referred Patient.
  • Eligibility: The Referred Patient must be a new patient who has not attended Smile Doctor previously. The physical referral card must be presented at the time of the appointment.

3. The Ambassador Offer ($200 Off Next Preventative Care Appointment)

  • Definition: "Preventative care appointment" refers to standard active maintenance, which may include a periodic examination (Item 012), removal of plaque/calculus (Item 114), and oral hygiene instruction (item 141) subject to clinical indication.
  • Credit Application: The $200 will be applied as a credit to the Ambassador’s patient file only after the Referred Patient has attended and completed their first appointment.
  • Usage: The $200 credit will automatically be applied against the total cost of the Ambassador's next clinically indicated preventative care appointment. If the appointment cost is less than $200, any remaining balance will be left on the account for future clinical use.

4. Clinical Discretion & Patient Safety (AHPRA Compliance)

  • Clinical Necessity: All treatments, including comprehensive exams and preventative care, are strictly subject to the clinical judgment of the treating dentist. If a specific service is not clinically indicated or in the patient’s best health interest, it will not be performed.
  • No Unnecessary Services: This offer is designed to facilitate access to ongoing oral health care and does not encourage the use of unnecessary health services.
  • Disclaimer: Any surgical or invasive procedure carries risks. Before proceeding, you should seek a second opinion from an appropriately qualified health practitioner.

5. Financial & Health Fund Conditions

  • Non-Transferable: Credits and discounts are non-transferable, cannot be exchanged for cash, and cannot be used to purchase retail products (e.g., electric toothbrushes, bleaching kits).
  • Private Health Insurance: If the patient has private health insurance, the discount/credit will be applied to the total account balance first. Health fund claims will be processed accurately reflecting the out-of-pocket costs actually paid by the patient, in accordance with standard health fund provider agreements.
  • Exclusions: This offer cannot be combined with any other promotions, discounts, or government-funded schemes (e.g., the Child Dental Benefits Schedule or DVA).

6. Validity

  • Smile Doctor reserves the right to alter, suspend, or terminate the Ambassador Club program at any time without notice. Any credits already applied to an Ambassador's account will remain valid for 12 months from the date of issue.
